New Patient Appointments New patients are asked to arrive 30 minutes prior to their scheduled appointment time in order to fill out the necessary paperwork to prepare the patient chart. These forms may be downloaded and completed prior to your arrival. Please complete the forms fully and bring them to your office appointment along with your insurance information.
Patient Information Form

Certain patients, depending on their medical condition, may be requested to take 2 plain Fleet enemas 1 hour prior to coming to their appointment in order to be prepared for any examination that may be necessary at that visit. The patient will be notified of this by the office staff making the appointment or by phone call prior to the visit date. If the patient is in a lot of pain, they may come without the enemas. If they are coming for a colonoscopy consultation, enemas are not necessary.
Office Hours Our office is open from 9:00 a.m. until 5:00 p.m. Monday-Friday. There is at least one physician on call at all times. If you have an urgent problem and it is necessary for you to reach the physician on call when the office is closed, please call our main phone number 704-333-1259, and you will be directed to call our answering service who will contact the physician on call. If you are expecting a call from a physician, please turn off your caller ID call blocking so you can receive a call from a private number.
Cancellations We ask that patients give at least 24 hours notice if they need to cancel or reschedule an office appointment. Surgical procedures and/or colonoscopies require at least 2 days notice as this affects other patients and their arrival times at the hospitals.
Prescription Refills Because patient records are unavailable after hours, ALL requests for pain medication or other medications MUST be called in Monday-Friday between 10:00 AM and 4:00 PM.
